I noticed at one point my ears were slightly larger and could fit bigger IEM sleeves but now I have to use the smallest silicone sleeve in each new set that I buy. I guess the human ear eventually starts shrinking after getting used to a smaller pair being inside the ear alot. The shrinking started after using my Klipsch Image S3's and their small cone shaped design,they are quite small and my ears got accustomed to them,then my other pair of Air Cushions which I used to use a bigger silicone sleeve with didn't fit anymore and hurt my ears.

Different IEM's will penetrate the ear different depths. And your ears do change, when getting custom fitted IEM's some companies suggest getting remoulds every 4 years. That doesn't mean they always get smaller, each person is different.
The diameter of the audio channel can greatly effect how an IEM will fit also. Some IEM's have a very small audio channel while others are fairly large. The size of this will effect how the silicone flanges will act in your ears, with a smaller canal you can use a larger flange and it will allow more flex.
