I got some made by a general audiologist. They made a mold of my ear, and instead of sending it away to be cast from plastic with the earphone inside the mold, they just used the original (silicon) mold and cut a hole for the headphone (in my case er6i's). Works alright, but doesn't like quite as nice as the professional versions.
Another option is to find a business that specializes in hearing aids, and seeing if they can do it cheaply.
